New Patients

To enhance your experience visiting the Haben Practice for Voice, please take a few minutes to review our billing and insurance information, policy regarding medical records and consider filling out the mandatory medical questionnaire online so that it may be transmitted and up loaded into your electronic medical record in advance of the time of your visit. This will also give you the opportunity to become familiar with our patient portal (Coming Soon). The portal is a secure online access to your personal medical information through this office 24 hours a day. Via the patient portal, patients have the ability to schedule appointments, communicate with the physicians, nurses and staff, as well as request medication refills and a number of other-related activities. Filling out the questionnaire online will save you from having to fill out a hard copy at the time of your check in.

Out-of-town clients

Many clients become aware of Dr. Haben's reputation regarding laryngeal cancer and laser endoscopic organ-preserving resections once they have already been diagnosed and are searching the Internet for other options. No definitive consultation or opinion can be rendered over the telephone, however Dr. Haben, as a courtesy, is willing to review a case and determine if an in-person consultation is warranted. Please contact Sandy at (585) 442-1110 and be prepared to forward all medical records, pathology slides and relevant scans on CD. Only once these have been reviewed, Dr. Haben will determine whether or not an in-person consultation is warranted.

Billing & Insurance

Billing service:

General information regarding medical insurance:
As a courtesy to our patients, Haben Practice for Voice will work with insurance providers with whom we have contracts. If your insurance plan is not on our list, we will be happy to assist you in obtaining a pre-certification from your insurance company for an out-of-network visit.

Your role in working with your insurance carrier:
Please bring all insurance cards with you at the time of your appointment. This will help you receive optimal coverage from your insurance carrier.

Our role in working with your insurance carrier:
If this is your first visit to Haben Practice for Voice, one of our staff members will call you prior to your visit to request insurance verification information from you so that pre-certification of either in-network or out-of-network coverage can be completed prior to your appointment.

Claims and payment policy

Co-pay and deductible amounts are expected at the time of service. We accept all major credit cards, cash and personal checks.

If Haben Practice for Voice is out of your insurance plan network, it is typical that your insurance will pay a smaller percentage of charges leaving a larger percentage of those charges as the responsibility of the guarantor.

Uninsured patients will be asked to pay in full at the time of service. Service will be discounted for all self-pay patients.

Medical Records

Any record created at the Haben Practice for Voice & Laryngeal Laser Surgery will be available 24 hours a day, 7 days a week via our patient portal (Coming Soon). If you are a new patient, and have records pertinent to your visit, you will need to request these records in advance of your appointment from their place of origin. Alternatively, we can assist you in this request by having you sign a medical release form, and your records will be obtained on your behalf.
